Medication procedures at school
All medications must be stored and administered in the school nurse's office. Only prescription medications that are in the original container and properly labeled may be given by school personnel. Over the counter medications may not be given unless accompanied by a note from a physician. Students may not hand carry or self administer medications. For the safety of all students, medications should be brought to the school by a parent and a medication permission form completed with instructions about dosage and times to be given.
Parent Medication Permission Form
If your child needs any prescription medication at school, please bring this completed form to the school nurse with the medication.
Physician Medication Form
Print this form and take it with you to your child's Dr. appointment for any non-prescription or OTC medications needed at school.
Permission to Carry Prescription Asthma Medication at School
This form must be filled out and signed by the physician and parent in order for a student to carry and self administer prescription asthma medication at school. Please bring this form to the school nurse when completed.
