Use this form to request tech items. This includes both hardware and software. To enable us to track the status of each item requested, please list only one item per request.

Enter your network id. This is your last name for most of you. It is not your teacher id number in GradeSpeed.

Select the item requested from the list. Choose "Other" for unlisted items.

Include the name of the item if you checked "Other" above. For all items, include a description if the name might not be enough.

Indicate whether the item is a additional item or a replacement for an existing item.

Explain why you want or need the item. Information provided may be used to establish priority.
