Necessary forms for athletics |
To practice, an athlete must have ALL forms in to Pete Wesp. An athlete is not considered cleared until Coach Wesp has received and acknowledged receipt of the forms.
The forms required for athletic participation are as follows:
Physical filled out by your physician
Medical History (form updated 1/06/09)
Acknowledgement of Rules
Steroid Waiver (form updated 2009)
Travel/Medicine Release for 09/10 school year
Physicals for the 2009-2010 school year have to be dated April 30, 2009 or later.
Again, please remember ALL of the above forms must be turned in before the student athlete can practice with the sports teams.

Athletes and Parents: please make sure to use the 1-06-09 version of the Medical History form that is part of this download. I cannot accept any other version and will request that you fill out the form again if the wrong one is turned in. Thank you,

Injury documentation
In order to provide the best care possible for the student athlete, the Athletic Trainer must be kept abreast of all illnesses and injuries. Should your son or daughter go to the doctor please bring a note back with directives from the doctor about play guidelines. If a student athlete does not bring a note back, it will be assumed that the doctor said he/she was not allowed to play and the student will sit out until the note is received.
